Terms and Conditions

Use of the Putih Dental Website, Mobile Web, and Application · Version 1.0 · Effective July 27, 2026 · PT Putih Dental Berkat

1. Introduction

Welcome to Putih Dental.

These Terms and Conditions (“Terms”) govern the use of the Putih Dental Website, Mobile Web, and Application (“Platform”) owned and operated by PT Putih Dental Berkat, including all of its branches and affiliates (“Putih Dental,” “We,” or “Us”).

By creating an account, accessing or using the Platform, registering, making an appointment, or using any other services through the Platform, you acknowledge that you have read, understood, and agreed to all provisions of these Terms.

By using the Platform, Users understand that personal data and health data will be processed in accordance with Putih Dental’s Privacy Policy. Users provide consent, to the extent required by applicable laws and regulations, for the processing necessary to provide the services.

Users agree that any consent, confirmation, or other actions performed through the Platform have the same legal force as written consent to the extent permitted by applicable laws and regulations.

If you do not agree to part or all of these Terms, you are not permitted to use the Platform or any services available through it.

2. Definitions

In these Terms, the following terms have the meanings set forth below:

Platform means the website, mobile web, iOS application, Android application, and/or other digital systems provided by Putih Dental.

Patient means any person who uses Putih Dental’s services, whether or not such person has undergone any medical procedure.

Account means a digital identity created by a User to access the Platform.

Appointment means a reservation for a consultation or medical procedure.

Services means all dental and oral healthcare services provided by Putih Dental.

Doctor means a dentist, dental specialist, or other healthcare professional cooperating with Putih Dental.

Medical Record means all patient medical information in accordance with applicable laws and regulations.

3. User Requirements

Users must:

  • be at least 18 years old; or
  • if under 18 years old, use the Platform through a parent or legal guardian.

Users must provide information that is:

  • true;
  • accurate;
  • complete;
  • not misleading; and
  • kept up to date whenever changes occur.

Putih Dental has the right to reject, suspend, or delete an Account if any information is found to be inaccurate.

4. Account Registration

To use certain features, Users are required to create an Account.

Users are responsible for maintaining the security of:

  • passwords;
  • OTPs;
  • verification codes; and
  • devices.

All activities performed using a User’s Account are deemed to have been performed by the owner of that Account.

If there is any suspected misuse of an Account, the User must immediately contact Putih Dental.

5. Use of the Platform

Users agree to use the Platform only for lawful purposes.

Users are prohibited from:

  • using a false identity;
  • impersonating another person;
  • interfering with the Platform system;
  • hacking;
  • distributing malware;
  • accessing other Users’ data without authorization; or
  • using the Platform for unlawful activities.

Putih Dental has the right to restrict access if misuse is identified.

6. Appointment Booking

The Platform enables Patients to:

  • book consultations;
  • book procedures;
  • select a branch;
  • select a Doctor;
  • select a date; and
  • select an available time.

An Appointment is considered confirmed only after the Patient receives confirmation through the Putih Dental system.

Putih Dental has the right to change an Appointment schedule in certain circumstances, including but not limited to:

  • the Doctor being unavailable;
  • a medical emergency;
  • operational disruptions; or
  • force majeure.

If the Doctor is unavailable as referred to in these Terms, Putih Dental may replace the Doctor with another Doctor with appropriate qualifications after informing the Patient. In such circumstances, Putih Dental will contact the Patient to reschedule the Appointment.

7. Cancellation and Rescheduling

Patients may cancel or change their Appointment in accordance with Putih Dental’s policies.

If a Patient:

  • arrives late;
  • fails to attend (no-show); or
  • repeatedly cancels Appointments,

Putih Dental has the right to:

  • restrict online booking features;
  • require a deposit for subsequent Appointments; or
  • adjust the schedule based on availability.

8. Patient Arrival

Patients are expected to arrive at least 15 minutes before their scheduled Appointment.

Late arrival may reduce consultation time or result in the Appointment being rescheduled if it interferes with the schedules of other Patients.

9. Cost Estimates

All costs displayed through the Platform are estimates.

The final cost may change based on:

  • the results of the Doctor’s examination;
  • clinical condition;
  • the need for additional procedures;
  • materials used; or
  • supporting examinations.

The Doctor has professional authority to determine an appropriate treatment plan based on the Patient’s condition.

10. Payment

Payment may be made through the payment methods provided by Putih Dental.

If the Platform provides electronic payment facilities, the User agrees that the transaction will be processed by a payment service provider cooperating with Putih Dental.

All transactions are subject to the terms and conditions of the relevant payment provider.

11. Membership and Loyalty Programs

Putih Dental may provide the following programs:

  • Membership;
  • Loyalty Program;
  • Vouchers;
  • Promotions;
  • Cashback; and
  • Referral Program.

Each program has its own terms and conditions. Such programs may not be combined, exchanged for cash, and/or transferred in any other form.

Putih Dental has the right to change, discontinue, or limit any program at any time.

12. Medical Records

All patient medical information is managed in accordance with applicable laws and regulations.

Medical Records include, among others:

  • examination results;
  • diagnoses;
  • clinical photographs;
  • intraoral photographs;
  • extraoral photographs;
  • radiographs;
  • panoramic images;
  • CBCT scans;
  • digital scan results (such as iTero);
  • digital models;
  • laboratory results; and
  • Doctor’s notes.

Medical Records are owned by the healthcare facility in accordance with applicable laws, while Patients have rights to the contents of their Medical Records in accordance with applicable laws and regulations.

13. Medical Procedures

Information available on the Platform is intended to provide general education.

Such information does not replace:

  • direct examination;
  • diagnosis;
  • professional consultation; or
  • an individualized treatment plan.

Medical decisions are entirely within the authority of the Doctor based on clinical examination.

14. Electronic Communications

By using the Platform, Patients agree to receive communications from Putih Dental through:

  • WhatsApp;
  • SMS;
  • email;
  • telephone;
  • push notifications; and
  • in-app notifications.

Communications may include:

  • Appointment reminders;
  • examination results;
  • treatment follow-ups;
  • administrative information;
  • membership information; and
  • promotions, where the Patient has provided consent.

15. Intellectual Property Rights

All contents of the Platform, including but not limited to logos, designs, photographs, videos, articles, illustrations, software, source code, and trademarks, are owned by Putih Dental or by parties granting licenses to Putih Dental.

Users are prohibited from reproducing, distributing, modifying, or using such content without written permission.

16. User Responsibilities

Users are responsible for:

  • the accuracy of the information provided;
  • use of their Account;
  • password security; and
  • decisions regarding the use of the Platform.

Users are responsible for any losses arising from inaccurate information.

17. Putih Dental’s Responsibilities

Putih Dental endeavors to provide the Platform optimally.

However, Putih Dental does not guarantee that the Platform will always be:

  • free from disruptions;
  • free from errors; or
  • available at all times.

Putih Dental has the right to conduct system maintenance, which may temporarily make the Platform inaccessible without prior notice, as part of its efforts to provide an optimally functioning Platform for Patient convenience.

Putih Dental is not responsible for losses arising from:

  • internet disruptions;
  • failure of the User’s device;
  • third-party errors;
  • force majeure; or
  • misuse of an Account by the User.

18. Limitation of Liability

To the extent permitted by applicable law, Putih Dental shall not be liable for indirect, incidental, consequential, loss of profit, loss of data, or other losses arising from the use of the Platform, except where such losses are caused by the fault or negligence of Putih Dental as determined under applicable law.

Putih Dental has the right to refuse or terminate services if a Patient engages in conduct that threatens the safety of healthcare professionals, staff, or other Patients, or violates applicable provisions, without prejudice to the Patient’s rights under applicable laws and regulations.

Nothing in this document limits any rights of Patients granted by applicable laws and regulations.

19. Force Majeure

Putih Dental shall not be responsible for delays or failures in providing services caused by circumstances beyond its reasonable control, including but not limited to:

  • natural disasters;
  • pandemics;
  • war;
  • national system disruptions;
  • power outages;
  • telecommunications disruptions; or
  • government policies.

20. Account Suspension and Deletion

Putih Dental has the right to suspend or delete a User’s Account if the User:

  • violates these Terms;
  • engages in unlawful activities;
  • provides false data;
  • misuses the Platform; or
  • interferes with system security.

21. Changes to the Terms

Putih Dental may amend these Terms from time to time to accommodate developments in services, technology, or applicable laws and regulations.

The latest version will be published through the Platform and will take effect from the effective date stated therein.

22. Governing Law and Dispute Resolution

These Terms are governed by and construed in accordance with the laws of the Republic of Indonesia.

Any dispute arising from these Terms shall first be resolved through deliberation to reach a mutual agreement.

If no resolution can be reached, the dispute shall be resolved in accordance with the mechanisms provided under the laws and regulations of the Republic of Indonesia.

1. Introduction

PT Putih Dental Berkat, together with its branches, affiliates, and entities under the same management (“Putih Dental,” “We,” or “Us”), is committed to protecting the privacy and security of the personal data of every Patient, User, website visitor, and user of the Putih Dental application (“You”).

This Privacy Policy explains how Putih Dental collects, uses, stores, discloses, protects, and processes Your Personal Data when You use our website, mobile web, application, or healthcare services.

Use of the Platform is also subject to the Putih Dental Privacy Policy, which forms an integral part of these Terms and Conditions.

By using the Platform or Putih Dental’s services, You acknowledge that You have read, understood, and agreed to this Privacy Policy.

2. Legal Basis

Personal Data Processing is carried out with reference to the laws and regulations of the Republic of Indonesia, including but not limited to:

  • Law Number 27 of 2022 concerning Personal Data Protection;
  • Law Number 17 of 2023 concerning Health;
  • regulations concerning Medical Records;
  • regulations concerning the Operation of Electronic Systems; and
  • other applicable regulations.

If there are changes to applicable regulations, Putih Dental will adjust this Privacy Policy in accordance with the applicable provisions.

3. Definitions

Personal Data means any data concerning an individual who can be identified, directly or indirectly.

Specific Personal Data means data that is sensitive under applicable laws and regulations, including health data.

Platform means the Putih Dental website, mobile web, iOS application, Android application, and digital systems.

Data Processing includes the collection, recording, storage, use, analysis, disclosure, transfer, deletion, and/or other actions involving Personal Data.

4. Data We Collect

We may collect the following Personal Data:

  • Full name;
  • National Identity Number (where required);
  • Passport/KITAS number (for foreign Patients where required);
  • Date of birth;
  • Gender; and
  • Nationality.

B. Contact Data

  • Telephone number;
  • WhatsApp;
  • Email;
  • Residential address; and
  • Emergency contact.

C. Account Data

  • Username;
  • Encrypted password;
  • OTP;
  • Login history;
  • Device ID; and
  • Notification token.

D. Health Data

As a healthcare facility, Putih Dental processes Specific Personal Data in the form of health data, including but not limited to:

  • Medical history;
  • Disease history;
  • Allergy history;
  • Medication history;
  • Procedure history;
  • Diagnosis;
  • Odontogram;
  • Periodontal chart;
  • Treatment plan;
  • Doctor’s notes;
  • Prescriptions;
  • Laboratory results;
  • Consultation results; and
  • Visit history.

E. Clinical Data

Including but not limited to:

  • Intraoral photographs;
  • Extraoral photographs;
  • Smile photographs;
  • Facial photographs;
  • Clinical videos;
  • Periapical radiographs;
  • Panoramic images;
  • Cephalometric images;
  • CBCT scans;
  • Intraoral scan results;
  • Digital models;
  • STL files;
  • iTero results; and
  • Procedure documentation.

All such data is treated as part of the Medical Record in accordance with applicable laws.

F. Payment Data

  • Payment method;
  • Payment amount;
  • Transaction status;
  • Invoice; and
  • Payment history.

Putih Dental does not store full credit card data where transactions are processed by a third-party payment service provider.

G. Technical Data

  • IP address;
  • Browser;
  • Operating system;
  • Device model;
  • Activity logs;
  • Cookies;
  • Crash reports; and
  • Analytics.

5. How We Collect Data

We obtain Personal Data through:

  • Account registration;
  • Appointment booking;
  • Registration forms;
  • consultations;
  • medical procedures;
  • communication via WhatsApp;
  • email;
  • telephone;
  • website;
  • application;
  • surveys;
  • customer service;
  • social media;
  • referrals; and
  • parties lawfully authorized by You.

6. Purposes of Data Processing

We use Personal Data for:

Healthcare Services

  • Patient registration;
  • Patient identification;
  • preparation of Medical Records;
  • diagnosis;
  • treatment;
  • treatment follow-up; and
  • management of follow-up schedules.

Administration

  • identity verification;
  • payment;

  • Appointment reminders;
  • schedule-change notifications;

  • membership management;
  • loyalty programs;

  • referral programs.

  • training of medical personnel, with appropriate safeguards; and
  • internal research in accordance with applicable legal and ethical requirements.

Legal Compliance

  • protecting and defending Putih Dental’s legal rights.

7. Consent

Where required by applicable laws and regulations, Putih Dental will obtain Your consent before processing certain Personal Data.

You may withdraw Your consent to the extent that such withdrawal does not conflict with Putih Dental’s legal obligations to retain or process certain data.

If it is necessary to use service providers located outside the territory of Indonesia, Putih Dental will ensure that Personal Data transfers are carried out in accordance with the requirements stipulated by applicable laws and regulations, including the implementation of adequate protection.

Putih Dental implements reasonable technical and organizational measures to protect Personal Data, including:

  • appropriate data encryption during transmission;
  • access restrictions based on authorization;
  • user authentication;
  • system activity logging (audit logs);
  • firewalls and network protection;

  • system security monitoring; and

Nevertheless, no electronic system can be guaranteed to be 100% free from security risks.

11. Data Retention

Personal Data is stored for as long as necessary to:

  • provide healthcare services;
  • fulfill legal obligations;
  • comply with requirements concerning Medical Record retention;
  • resolve disputes; and
  • conduct audits.

After the applicable retention period expires, Personal Data will be deleted, destroyed, or anonymized in accordance with applicable laws and regulations.

12. Your Rights

In accordance with applicable laws and regulations, You may submit requests to:

  • obtain information regarding the processing of Personal Data;
  • access Personal Data;
  • correct inaccurate Personal Data;
  • update Personal Data;
  • withdraw consent where consent is the basis for processing;
  • request restriction of processing under certain circumstances;
  • object to certain processing in accordance with applicable laws;
  • obtain a copy of Personal Data where possible; and
  • submit complaints to Putih Dental or the competent authority in accordance with applicable laws and regulations.

Requests will be processed after identity verification and provided that they do not conflict with Putih Dental’s legal obligations.

13. Medical Records

Medical Records are governed by applicable healthcare laws and regulations.

Putih Dental is required to retain Medical Records for the retention period stipulated by applicable laws and regulations.

Requests for access to the contents of Medical Records will be processed in accordance with Putih Dental’s internal procedures and applicable laws and regulations.

14. Communications and Marketing

With Your consent where required, Putih Dental may send information regarding:

  • promotions;
  • membership programs;
  • dental health education;
  • periodic examination reminders; and
  • Putih Dental activities or events.

You may stop receiving marketing communications at any time through the mechanisms provided. Opting out of marketing communications does not affect the delivery of administrative communications or communications related to healthcare services.

15. Cookies and Similar Technologies

The Putih Dental website and application may use cookies or similar technologies to:

  • remember User preferences;
  • maintain login sessions;
  • improve security;
  • analyze Platform usage; and
  • improve User experience.

You may manage cookie usage through Your browser settings; however, certain Platform functions may not operate optimally if cookies are disabled.

16. Third-Party Services

The Putih Dental Platform may connect with third-party services, including but not limited to:

  • Google Maps;
  • Apple Maps;
  • payment gateways;
  • WhatsApp;
  • analytics services; and
  • push notification providers.

Use of such third-party services is subject to the respective privacy policies of those providers.

17. Minors

Putih Dental services are also used by child Patients.

Where a child’s Personal Data is processed, the required consent and actions will be carried out by a parent, guardian, or other authorized party in accordance with applicable laws.

18. Artificial Intelligence (AI)

Putih Dental may use Artificial Intelligence (AI)-based technologies to support operations, improve services, analyze data, provide customer services, or perform other functions.

The use of AI is intended as a supporting tool and does not replace the professional judgment of Doctors. Medical decisions remain the responsibility of authorized medical professionals based on examinations and clinical considerations.

If Personal Data is used in the development or evaluation of AI systems, Putih Dental will implement appropriate safeguards in accordance with applicable laws and data security principles.

19. Changes to the Privacy Policy

Putih Dental may amend this Privacy Policy from time to time.

The latest version will be published through the Platform together with its effective date.

Material changes will be communicated through media considered reasonable by Putih Dental.

20. Miscellaneous

These Terms and Conditions together with this Privacy Policy constitute the entire agreement between the User and Putih Dental regarding the use of the Platform.
